- Super fund account number and fund name
- Most recent annual super statement showing TPD cover details
- Product Disclosure Statement (PDS) for your super fund
- Insurance policy schedule (if available)
- Claim forms provided by the fund (requested from the fund)
Medical evidence
- Specialist reports from each treating specialist — diagnosis, treatment, prognosis
- GP medical records covering the period since your condition began
- Hospital discharge summaries or procedure reports
